Community Living Supports (CLS)
The Community Living Support Program is designed to enable individuals to lead an independent life in a community setting. A Better Life Together, Inc provides qualified, trained staff, that are selected based upon the needs and desires of the person receiving services. Currently we serve individuals throughout region 3 in this program.
Community Living Support (CLS) is regularly scheduled services that assist participants in maintaining a healthy and productive life at home and in the community. Services include activities of daily living, communication building, self-medication, and money management, which are provided by trained and experienced staff.
Who is Eligible for Community Living Support Services?
To qualify for acceptance into the program, individuals must have a diagnosis of either: Traumatic Brain Injury, Intellectual Disability or Developmental Disability from a Medical Professional. The funding for this program comes from a variety of sources including: grants, Intellectual or Developmental Disability wavier services, and Family Support funding.
What does the Community Living Support Program Entail?
First, most of our participants live in their own homes. A Better Life Together, Inc staff provides assistance with activities of daily living such as: bathing, grooming, cooking, housekeeping, personal care, assistance in community activities and learning social skills. The hours of service are based on the individuals needs and will be provided with a minimum of interference with their daily life.
Community Living Support Services are designed for people who can accomplish quite a few things on their own, but who may need assistance in a certain aspect of their daily living. Most people receiving CLS Services only receive up to 6 hours of service a day. It truly depends on the need of the person.
Examples:

Social, leisure, and adaptive skill development that assists the individual in continuing home living.

Personal care, protective oversight and supervision as a component part but not as the entirety of the services.

Training in activities of daily living, such as bathing, dressing, toileting, transfers, hygiene, light housework, laundry, meal preparation, grocery shopping, using telephone, medication and money management

Medically related services, such as basic first aid, arranging, accompanying, providing individual transportation to medical appointments, and reminders and/or assistance self-administration of medication.

Implementation of a behavioral support plan to reduce inappropriate behaviors and to acquire new skills and behaviors.

Transportation to grocery or personal shopping, banking, and other community activities supporting continued home living.

CLS RN and LPN services which are defined as only routine nursing services that are integral to meeting the daily needs of the person in his/her own or family home.
